Permission Slips

Additionally, please review and respond to the Internet Permission, Permission to Photograph, Walking Permission, and Family Handbook by going to Web Forms in your FACTS Family Portal. 

1. Click here to log in to FACTS Family Portal
2. Enter the District Code “DW-NY”
3. Enter your FACTS Username and Password
4. Click on Web Forms
5. Click the “Family Demographic Form”
6. Click on the “Back to School Forms 2023” next to a student's name
7. Click the link in the form description to access the permission slips and review them
8. Complete the form and submit
9. If you have more than one student, repeat for each student
